Division of assets:
Pick the one that best fits your situation. More detail about assets will be filled out later.
Both parties agree what the division of assets should be: I keep everything I have in my possession and the other party keeps everything s/he has in his/her possession.
Both parties agree what the division of assets should be: I plan to provide a complete list of what each person will get at the beginning of the case.
I am fairly certain about what the division of assets should be, but the other side may not agree with me. I plan to provide a complete list of what I think each person should get at the beginning of the case.
I am not sure what the division of assets should be but want to get the paperwork started and file the case now. I will decide about asset division, debt allocation, and other matters in more detail after the case is filed.
The following section outlines assets that are involved in this case. Complete the section regardless of whether you and the other party have agreed to the asset division. Select each that applies or may apply to this case.

I have a list of specific furniture, clothes, and household items each person should get or keep and I can provide it at the beginning of the case
Each person should keep the furniture, clothes, and household items in his or her current possession
I do not know how the furniture, clothes, and household items should be divided and want to decide at a later date or have the court to decide at a later date
I want to discuss options regarding the furniture, clothes, and household items

Other Provisions
I would like to include the following in the parenting plan:
Each parent shall have the right to telephone, text, video conference, and Email contacts with the child while the child is away from that parent at reasonable times and for reasonable durations.
If the child is traveling and will be absent from the then residential parent's home for more than 24 hours, the residential parent shall inform the other parent of all travel plans including destination, dates of travel, mode of travel, airline information, contact number and address, etc. as soon as the travel plans are made or at least 14 days prior to the travel. If the child is camping with the residential parent, a general location shall suffice for an address. If this information is not provided as described, the non-residential parent may withhold the child from travelling until the information is provided.
Each parent shall be responsible for keeping themselves advised of school activities, athletic, and social events in which the child participates. Both parents may participate in school activities for the child such as an open house, attendance at an athletic event, etc.
Neither parent shall make disparaging remarks about the other parent to or in front of the child or where the child may overhear. Both parents shall cooperate in fostering a good and loving relationship between the child and the other parent.
